By booking your polygraph examination with Polygraph Professional Services LLC you are agreeing to the following:

• The full price of one polygraph examination is $700, with a double appointment being $1200.

• The appointment will take place at 9861 Broken Land Parkway, Columbia, MD 20146, in Suite 100. Please check in with my receptionist upon arrival.

• -The $700 or $1200 (depending on appointment type) is non-refundable. 48 hours are needed for a reschedule. By not following any of the protocol necessary, the polygraph will not be conducted and no refund will be issued.

• I do not test anyone who is pregnant

• The person taking the test is not under threat of domestic violence and does not have any current suicidal ideations or behaviors. 

• No illegal drug use or alcohol consumption for a minimum of 24 hours prior to scheduled testing (this includes marijuana)

• Please take your prescription medication as prescribed

• Children are not allowed at the testing facility for any reason

• Arrive 15 minutes early for testing, and text me when you arrive at the facility.

• If you arrive for the exam late, you are agreeing to pay $25/every 5-minute duration the polygraph examination is delayed. After 15 minutes, your appointment will be cancelled (you will not be issued a refund and will be charged the remaining $200)

• Please wear comfortable clothing to testing

• Cell Phones must be turned off when entering the facility (unless instructed otherwise by the examiner)

• The examiner has the right to terminate the polygraph exam without a refund at any time due to inappropriate or unlawful behavior.
