By booking your polygraph examination with Polygraph Professional Services LLC you are agreeing to the following:

The full price of the polygraph examination is $475. The scheduling Deposit of $275 is necessary to solidify the date & time of your appointment and the remaining $200 is due when you arrive for testing. The scheduling deposit of $275 is NON-REFUNDABLE and 48 hours notice prior to scheduled testing date & time is needed for cancellation or you will be charged the remaining $200. In addition, "No call, no show" will be charged the remaining $200. By booking this polygraph examination you are agreeing to the following: 

• I do not test anyone who is pregnant

• No illegal drug use or alcohol consumption a minimum of 24 hours prior to scheduled testing (this includes marijuana)

• Please take prescription medication as prescribed

• Children are not allowed at the testing facility for any reason

• Arrive 15 minutes early for testing at text me when you arrive at the facility.

• If you arrive for exam late, you are agreeing to pay $25/every 5 minute duration the polygraph examination is delayed. After 15 minutes, your appointment will be cancelled (you will not be issued a refund and will be charged the remaining $200)

• Please wear comfortable clothing to testing

• Cell Phones must be turned off when entering the facility (unless instructed otherwise by the examiner)
